  • Reduced palatability of the restricted foods. Eliminating biscuits, crisps, pastries, and sugary drinks removes many of the most hyper-palatable foods in a typical diet, which can reduce overall consumption independent of macronutrient effects.

What the research shows at 12 months and beyond

Short-term trials (up to 6 months) consistently show greater weight loss with low-carb diets compared to low-fat or higher-carb diets. At 12 months, the difference typically narrows substantially, and by 2 years most well-controlled trials show comparable results between dietary patterns. The leading explanation is adherence: low-carb diets produce faster initial loss, but maintaining very low carbohydrate intake long-term is difficult for many people, and as adherence degrades, so does the advantage.

This does not mean low-carb diets do not work long-term — for people who find them genuinely sustainable, they can be an excellent long-term approach. It means that no dietary pattern has a permanent physiological advantage over all others for weight management; the best approach is the one you can actually maintain.

  • Blood glucose and insulin. Carbohydrate restriction directly reduces post-meal blood glucose spikes and lowers insulin levels. This is the primary clinical rationale for low-carb approaches in type 2 diabetes management, where the evidence is strong enough that clinical guidelines in many countries now include it as an option. Anyone with diabetes or prediabetes should discuss dietary changes with their doctor before implementing them.
  • Triglycerides. Carbohydrate restriction reliably reduces blood triglyceride levels, a cardiovascular risk marker.
  • HDL cholesterol. Low-carb diets typically raise HDL ('good') cholesterol.
  • LDL cholesterol. The effect on LDL varies and depends heavily on the types of fat replacing the carbohydrates. Replacing carbohydrates with saturated fat may raise LDL; replacing with unsaturated fat typically does not. This is worth monitoring.
  • Appetite. Many people report significantly reduced hunger on low-carb diets, likely due to higher protein intake and a reduction in blood glucose fluctuations.

Common challenges and how to address them

  • 'Keto flu' in the first 1–2 weeks. As glycogen depletes and sodium is excreted, fatigue, headaches, and brain fog are common. Adequate sodium, potassium, and magnesium intake, and drinking enough water, reduces the severity significantly.
  • Constipation. Lower fibre intake is common when grains and legumes are removed. Prioritising non-starchy vegetables, nuts, seeds, and — if not strictly ketogenic — lower-carb fruits helps maintain adequate fibre.
  • Social difficulty. Many social foods are carbohydrate-based. Flexible approaches that allow higher carbohydrate intake at social occasions are often more sustainable than strict adherence.
  • Dietary variety. A low-carb diet that relies heavily on a small number of foods tends to produce nutritional gaps and dietary monotony. Prioritising vegetables, variety in protein sources, and healthy fats maintains nutritional completeness.

Who tends to do best on a low-carb approach

Research and clinical experience suggest that low-carb approaches tend to work particularly well for people with insulin resistance or type 2 diabetes (where the glucose-lowering effect is directly therapeutic), people who find fat and protein more satiating than carbohydrates, people who struggle with energy fluctuations after high-carbohydrate meals, and people who are willing to substantially change their food habits rather than make small modifications across all food groups.

It tends to be less well-suited to people who are highly active or rely on carbohydrates for exercise performance, people who eat a predominantly plant-based diet (where many protein and fat sources are also carbohydrate-containing), and people who find the restrictions socially or psychologically difficult to sustain.
