The majority of early intermittent fasting research was conducted in men, or in mixed populations where male-specific responses dominated the analysis. As research has caught up, a clearer picture has emerged: intermittent fasting affects women differently from men, in ways that have practical implications for which protocols to choose, how long to fast, and how to recognise when a fasting approach is not working for your physiology.
This does not mean IF is unsafe or ineffective for women — the evidence supports meaningful benefits for many. It means women should approach fasting with specific knowledge, not with protocols designed for and tested primarily in men.
How hormonal biology creates different responses
Women's metabolic and reproductive systems are tightly regulated by the hypothalamus — the brain region that acts as a master controller for hormones including those that govern the menstrual cycle. Female rodents show this hypothalamic sensitivity more strongly and rapidly than males — within days of caloric restriction, reproductive cycling can be disrupted in rodent models. Human evidence is less clear-cut, but case reports and observational data suggest that some women experience menstrual irregularity when adopting aggressive fasting protocols, particularly OMAD or extended fasting on top of already-high training loads.
What specific changes women may notice
- Menstrual irregularity or missed periods — particularly with longer fasting windows (18+ hours) or if overall calorie intake is significantly reduced.

- Ensure adequate calorie intake over the week. Even if individual days are restricted, the weekly average should not fall into severe deficit for extended periods.
- Watch for warning signs. Missed periods, persistent fatigue, hair loss, or worsening mood are signals that the fasting protocol may be too aggressive for your physiology.
Fasting during different life stages
Pregnancy and breastfeeding: Standard IF protocols are not appropriate during pregnancy or breastfeeding. Adequate calorie and nutrient intake is critical for foetal development and milk production. Do not fast in these stages without direct medical guidance.
Perimenopause and menopause: The hormonal context changes significantly.
