BMI and Its Limits — What Body Mass Index Does and Does Not Tell You

BMI is widely used but widely misunderstood. Here is what it measures, what it misses, and what complementary measures give a more complete health picture.

Body mass index — weight in kilograms divided by height in metres squared — is the most widely used population-level measure of body weight relative to height. It appears on health questionnaires, insurance forms, clinical guidelines, and fitness apps. And yet it is also one of the most frequently misunderstood measurements in health, because what it measures and what it is used to imply are not always the same thing. Understanding its legitimate uses, its genuine limitations, and what to track alongside it makes it a more useful tool — and prevents it from being a source of unnecessary anxiety or false reassurance.

What BMI measures (and was designed for)

BMI was developed by Adolphe Quetelet in the 1830s as a statistical tool for characterising populations, not for assessing individual health. Ancel Keys later popularised it for use in epidemiological research on obesity in the 1970s. At a population level, BMI correlates reasonably well with body fatness on average, and population-level associations between high BMI and health outcomes are robust and well-established.

The standard categories — underweight (below 18.5), normal weight (18.5–24.9), overweight (25–29.9), and obese (30 and above) — were established from these population associations and are used in clinical and public health settings as convenient risk thresholds. They are not hard biological boundaries.

What BMI does not measure

  • Body composition. BMI does not distinguish between fat mass and lean mass (muscle, bone, organs, water). A person with substantial muscle — trained athletes, for instance — can have a high BMI while carrying relatively little body fat and having excellent metabolic health. Conversely, a person with very little muscle but substantial fat can have a 'normal' BMI while carrying metabolic risk.
  • Fat distribution. Where fat is stored matters considerably for health risk. Visceral fat (around internal organs) carries far higher cardiometabolic risk than subcutaneous fat. BMI captures neither where fat is nor how much of it is visceral.
  • Individual metabolic health. Some people with BMI in the 'overweight' range are metabolically healthy — good blood pressure, blood glucose, lipids, and inflammation markers. Others with 'normal' BMI have metabolic risk factors. BMI alone does not reliably distinguish these.
  • Ethnic variation. The standard BMI thresholds were developed primarily from European populations. Research shows that people of South Asian, East Asian, and some other ethnic backgrounds face elevated metabolic risk at lower BMI values — which has led some health authorities to recommend lower thresholds for these groups.

What to track alongside BMI

BMI is useful as a rough starting point and for tracking change over time, but several additional measures provide a more complete picture of body composition and health risk:

  • Waist circumference. A simple measure of central adiposity that correlates well with visceral fat and cardiometabolic risk, often independently of BMI. WHO thresholds: elevated risk at waist above 94 cm (men) / 80 cm (women); high risk above 102 cm (men) / 88 cm (women).
  • Waist-to-height ratio. Some research suggests waist-to-height ratio (waist in cm divided by height in cm) is a stronger predictor of cardiometabolic risk than BMI. A commonly used target is a ratio below 0.5 — keeping your waist circumference to less than half your height.
  • Body fat percentage. Measured through DEXA scan, bioelectrical impedance, or skinfold calipers, this directly quantifies fat versus lean mass. It is more informative than BMI for individuals, though measurement accuracy varies by method.
  • Metabolic markers. Blood glucose, HbA1c, blood pressure, lipid profile, and inflammatory markers provide the most direct evidence of metabolic health. These are assessed through clinical blood tests.

Use BMI as a trend tool, not a verdict

For the purpose of personal weight management, BMI is most useful as a consistent relative measure of your own change over time — not as an absolute assessment of your health status. A falling BMI over months reflects genuine change. What that BMI number means in absolute terms is a question for a clinical assessment, not a formula.
