Thyroid and Weight — Understanding Hypothyroidism and Weight Gain

Thyroid conditions require medical diagnosis and treatment

If you suspect you have a thyroid condition, the correct first step is to see a doctor for blood tests (TSH, and often T3 and T4). Do not attempt to manage a thyroid condition with lifestyle changes alone — hypothyroidism is typically treated with thyroid hormone replacement therapy, which requires prescription and monitoring. The thyroid gland produces hormones that regulate how quickly your body uses energy — its metabolic rate. When thyroid output falls below the level your body needs (hypothyroidism or an underactive thyroid), metabolism slows, energy expenditure falls, and several related systems are affected: cardiovascular function, body temperature regulation, bowel motility, cognition, mood, and the ability to maintain or lose weight.

How hypothyroidism affects weight

Weight gain in hypothyroidism occurs through two main mechanisms. First, reduced metabolic rate — the body is burning fewer calories at rest. Second, fluid retention — thyroid hormones influence how the body handles salt and water, and their deficiency leads to a type of swelling called myxoedema that can add several kilograms of fluid to body weight. The fat accumulation component of hypothyroid weight gain is typically modest (studies average 3–5 kg), while the fluid component can be more variable.

What makes this clinically important for weight management is that treating the thyroid condition with appropriate hormone replacement (typically levothyroxine) often resolves the fluid retention and normalises metabolic rate, making subsequent weight management substantially more tractable. Attempting to aggressively diet before the thyroid is treated is likely to be frustrating and less effective.

Subclinical hypothyroidism and the 'normal range' question

Some people have TSH levels that are elevated but within what laboratories classify as the 'normal range,' or have symptoms of hypothyroidism with TSH in range. This is an area of ongoing clinical debate. If you have symptoms consistent with hypothyroidism — unexplained weight gain, fatigue, cold intolerance, constipation, dry skin, hair loss, low mood — and blood tests have not yet been done, this is worth discussing with your doctor. If tests have been done and returned 'normal,' asking about the specific TSH value and whether treatment is indicated for your specific symptoms is reasonable.

Weight management once the thyroid is treated

Once hypothyroidism is appropriately treated and thyroid levels are in the optimal therapeutic range, weight management becomes more similar to weight management in the general population. The following points apply:

  • Treatment normalises but does not necessarily cause weight loss. Restoring thyroid hormone levels typically stops the specific weight gain caused by the condition and may cause some reduction as fluid is lost, but it is not a guaranteed weight-loss treatment on its own.
  • Metabolic rate may remain slightly lower. Some research suggests that even with adequate treatment, resting metabolic rate in some people with treated hypothyroidism may remain modestly below population averages. This is variable and not universal.
  • Standard evidence-based approaches apply. A sustained energy deficit, adequate protein, resistance training, sleep, and stress management are as relevant for people with treated thyroid conditions as for anyone else — and there is no evidence that a specific 'thyroid diet' is required.
  • Selenium and iodine are relevant nutrients. The thyroid requires adequate iodine to produce hormones and selenium for thyroid hormone metabolism. Deficiency of either can impair thyroid function. However, supplementing beyond dietary adequacy is not recommended without medical guidance.
