Weight Loss and Menopause — What Changes and What Helps

Menopausal symptoms, including weight changes, vary widely and may benefit from medical management — including hormone replacement therapy (HRT) in appropriate candidates. Speak with your GP or gynaecologist before making significant dietary or lifestyle changes, and specifically if you have concerns about cardiovascular health, bone density, or other menopausal symptoms.

Menopause — defined as twelve months after the last menstrual period — is associated with a constellation of hormonal changes that have well-documented effects on body weight, fat distribution, and metabolism. These effects begin during perimenopause (which can start years before the final period) and continue into postmenopause. Understanding them helps explain why weight management becomes more challenging in this life stage, and what adjustments to the usual strategies are supported by evidence.

What changes at menopause

The primary hormonal change is the substantial decline in oestrogen. This has several weight-relevant effects:

  • Fat redistribution. Oestrogen promotes peripheral fat storage (hips and thighs) and protects against central adiposity. As oestrogen falls, fat distribution shifts toward the abdomen — both subcutaneous and visceral. This shift occurs even without significant change in total body weight, and it matters because visceral fat carries higher cardiometabolic risk.
  • Reduced muscle mass. Oestrogen has muscle-protective effects. Its decline accelerates the age-related loss of muscle mass (sarcopenia), which reduces resting metabolic rate and changes body composition.
  • Sleep disruption. Menopausal symptoms — hot flushes, night sweats, mood changes — frequently disrupt sleep, raising appetite hormones and impairing dietary decision-making.
  • Insulin sensitivity changes. Research suggests insulin sensitivity decreases around menopause, partly independently of changes in body fat, increasing the risk of metabolic syndrome.
  • Appetite and satiety changes. Lower oestrogen is associated with reduced leptin sensitivity and changes in how the brain processes food reward signals, which may increase appetite.

What the evidence supports

  • Resistance training becomes the highest-priority exercise. The muscle-protective effects of resistance training are particularly important when oestrogen-mediated protection of muscle is declining. Two to three sessions per week targeting major muscle groups is well-supported.
  • Higher protein intake. The evidence for higher protein intake during weight loss in older women — around 1.2–1.6 g per kg of body weight — is strong, supporting both satiety and the preservation of muscle mass during a caloric deficit.
  • Prioritise sleep quality. Menopausal sleep disruption directly affects appetite hormones. Managing symptoms that disrupt sleep — which may include clinical management of hot flushes with HRT or other approaches — is a practical step in weight management, not merely a quality-of-life issue.
  • Dietary quality over quantity. The Mediterranean dietary pattern — high in vegetables, fish, whole grains, olive oil, and legumes; low in processed foods — has the strongest evidence for both cardiovascular health and weight management in postmenopausal women.
  • HRT and weight. The evidence does not support the concern that hormone replacement therapy causes weight gain; some studies show HRT may actually attenuate the fat redistribution toward the abdomen that occurs at menopause. This is a clinical decision to be made with a qualified doctor.

Weight fluctuations during perimenopause are significant and hormone-driven — cycles become irregular, fluid retention patterns change, and the scale may move considerably without corresponding changes in fat.
